Exclamation Output Surge

Is there any possible way that a 120/240VAC generator could have a catastrophic fault that would cause a high voltage spike across the line-line (240VAC) damaging all 240V electronics without causing any damage on the N-Line (120VAC) electronics?

This is just my opinion but no. Generators are for the most part governed by RPM as to the output at max. I really don't see a major spike on the 220 side happening. The 110 uses 1 of the line and the neutral wires. I guess it the one not used in the 110 connection just happen to fail somehow, but the chances are slim to none I would guess.
